Therapy Partner Solutions (TPS) is therapistowned and operated and has been supporting clinicians since 2016. Youll be part of the therapy specific team supporting Home and HeartHome Health Care Agency working within a collaborative multidisciplinary model. Care is coordinated patientcentered and teamdriven with shared accountability and open communication.

Youll work with a diverse caseload including a higherthanaverage postoperative orthopedic population.

Requirements for this position include the following:
- Graduate (current or pending) from an accredited physical therapy program
- Active or pending state physical therapist license
- Recent graduates encouraged to apply
- Annual continued education allowance
- In-house continued education courses and certifications

- Team-based learning sessionscomprisedof presentations on relevant topics and skill acquisition
