Job Summary:
The Virtual Care Coordinator plays a vital role in supporting virtual patient care activities and enhancing patient-focused care across the healthcare system. This role will be instrumental in telehealth remote patient monitoring and virtual hospital programs ensuring high-quality safe and efficient care while adhering to clinical and operational standards.
Key Responsibilities:
Perform quality assurance peer reviews and program evaluations; recommend improvements to workflows and quality metrics
Lead case reviews and collaborate on implementing solutions and monitoring outcomes
Triage and assign cases for virtual monitoring and observation
Develop and implement telehealth policies and workflows
Maintain operational databases for tracking performance metrics response times and quality outcomes
Train new staff on telehealth platforms workflows and documentation requirements
Serve as a liaison between physicians clinical staff and technology vendors
Monitor telehealth consults and follow-up visits using Epic
Ensure timely and smooth operation of telehealth sessions
Provide technical support to clinical staff and physicians ensuring patient safety and workflow efficiency
Escalate patient safety concerns and equipment issues when necessary
Submit utilization and assignment reports each shift
Maintain accurate documentation of patient behaviors interventions and safety events
Participate in meetings and team briefs for continuous improvement
Clinical and Patient Safety Duties:
Provide continuous remote observation and surveillance of patients
Monitor vital signs Foley catheter days and central line days to identify early warning signs
Escalate any changes in patient condition or behavior to the RN or physician promptly
Ensure patient privacy and HIPAA compliance at all times
Assist with interpretation services as needed
Required Skills and Competencies:
Strong interpersonal and communication skills
Ability to navigate multiple technology platforms simultaneously
Strong problem-solving prioritization and decision-making skills
Excellent written and verbal communication in English
High attention to patient safety quality and compliance
Ability to function effectively in high-acuity environments
Experience Requirements:
Minimum 3 years of direct patient care experience
Telehealth or remote patient monitoring experience
Experience with policy development and quality assurance programs
Familiarity with EMR/Epic systems
Experience in identifying patient safety risks and changes in clinical status
Collaboration with interdisciplinary teams in virtual care settings
Understanding of regulatory legislative and organizational policies affecting telehealth
Education:
Associate or Bachelors degree in a health-related field (Preferred)
