Key Responsibilities
Deliver a comprehensive and high-quality pharmaceutical service to patients and service users.
Support and develop ward-based pharmacy services including ward rounds Kardex reviews medicines reconciliation and patient counselling.
Ensure the safe cost-effective procurement storage and supply of medicines in compliance with legal quality and safety requirements.
Maintain accurate records relating to purchasing quality assurance compounding and dispensing activities.
Provide expert advice and guidance to medical nursing and multidisciplinary teams on the safe effective and economical use of medicines.
Deliver medicines information and counselling services to patients and their families as required.
Identify report and appropriately manage medication-related risks and incidents while actively contributing to medication safety initiatives.
Assist in the preparation of annual reports and other departmental documentation as required.
Ensure compliance with health and safety policies and procedures.
Participate in discharge planning for priority patients and liaise with community healthcare professionals to support continuity of care.
Contribute to the ongoing implementation of safe generic substitution practices.
Support the development and review of pharmacy policies and procedures under the guidance of the Chief Pharmacist ensuring adherence to best practice and legislative requirements.
Collaborate closely with medical nursing and allied health professionals to promote patient safety and optimize treatment outcomes.
Participate in multidisciplinary committees audits quality improvement initiatives and research projects as directed.
Assist the pharmacy team in the efficient delivery of departmental services and operational activities.
Foster positive working relationships within the pharmacy department and across the wider hospital.
Undertake additional duties and responsibilities as assigned by the Chief Pharmacist CEO or other authorized hospital representatives.
Requirements
Degree in Pharmacy from a recognised institution.
Current registration with the Pharmaceutical Society of Ireland (PSI).
Excellent verbal and written communication skills.
Strong time management and organisational abilities with the capacity to manage competing priorities effectively.
Ability to work collaboratively as part of a multidisciplinary healthcare team.
Demonstrated ability to use initiative and work independently when required.
