The Opportunity
The Resident Care Coordinator (RCC) reports to the Executive Director or Director of Care and supports the implementation and evaluation of resident care services in accordance with AgeCare policies and applicable provincial standards guidelines and regulations. The role is responsible for supporting the delivery and maintenance of a high standard of resident care.
The Resident Care Coordinator promotes teamwork and a positive workplace culture that reflects AgeCares guiding principles of Trust Respect Teamwork and Quality while ensuring the rights individuality and diversity of residents families and staff are respected.
What You Will Do
Provides support and guidance to Care staff to achieve and maintain quality care within a person-centred model.
Reviews trends related to incident reports and medication incidents and ensures appropriate corrective action is taken to prevent recurrence.
Participates in provincial Continuing Care Health Service Standard compliance processes including action plan development and follow-up.
Ensures critical/reportable incidents and allegations of resident abuse are reported and managed in collaboration with the Executive Director or Director of Care and the Community RN Case Manager including required PPC investigation processes.
Ensures residents are admitted in accordance with AgeCare policies and applicable provincial standards.
Oversees the resident admission process in collaboration with the Community Placement Coordinator including the appropriate selection of residents based on available beds safety roommate suitability and space considerations.
Ensures bed vacancies are filled within six working days and supports maintaining full occupancy of the facility.
Ensures annual Resident Care Conferences are scheduled.
Keeps the Executive Director or Director of Care and the facility management team informed of emerging issues and concerns.
What You Bring
Minimum Licensed Practical Nurse (LPN) registration in good standing with the applicable provincial regulatory body required
Experience as a Licensed Practical Nurse (LPN) or in another nursing designation with experience in a long-term care or continuing care setting preferred.
Demonstrated leadership experience with strong skills in supervision judgment and interpersonal relationships.
Experience caring for residents living with dementia and complex care needs.
Knowledge of provincial legislation regulations standards and best practices related to resident care and continuing care.
Ability to work collaboratively with residents families care staff and interdisciplinary teams.
Proficiency with electronic documentation and Microsoft Office applications is an asset.
Must be able to provide a current Criminal Record Check through the BC Criminal Records Review Program (CRRP) or be willing to obtain one.
