Whatyoulldo:
Supervise and supportPSWs and HSWsto ensure practicescomply withall legislated standards of practice as well as company policies and procedures.
Respond to and advise/resolve concerns from community staff patients/families and other team members.
Provide ongoing leadership through regular huddlesface to facemeetingsand one-on-ones.
Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
Engage patients in problem solving using a collaborative approach offering health teaching buildingrapportand trust toidentifypatient health care needs strengthscapacitiesand goals.
Engage staff in problem solving offering educationcoachingand mentoring in resolution of problems.
Maintain the knowledgeskillsand attitudes to provide safe efficient effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.
Whatyou bring:
Current RN or RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions
Minimum of 5 years experience as a practicing Registered Nurse or Registered Practical Nurse
Comprehensive patient health assessment skills are
Possess an understanding of the principles of community nursing practice
Proven experience leading and managing a team
Knowledge of clinical documentation standards and practice guidelines is
Excellent organizational interpersonal and teaching skills
Commitmentto professional development in clinical practice that is relevant to home health care
Effectivecritical thinking and problem-solving skills
Mustpossessa valid drivers license and access to reliable transportation
Able to provide a current clear VulnerableSectorScreening (VSS)
C.N.A.Certification as a Community Health Nurse(CCHN(C))is considered an asset
