Job Summary

POSITION PURPOSE
To coordinate resident care with staff teams to ensure the best health and wellbeing outcomes for residents.

KEY ACCOUNTABILITIES / DUTIES
OPERATIONAL RESPONSIBILITIES
> Lead an integrated team of professional care staff.
> Acknowledge and develop a positive relationship with each resident and their family respecting the dignity and privacy of each individual.
> Empower residents to make informed choices particularly in relation to daily routines.
> Provide assessment planning implementation and evaluation of clinical care in consultation with residents and/or their representatives and appropriate team members
> Provide best practice clinical care tailored to meet resident needs.
> Produce a high level of accurate comprehensive and timely documentation of resident care needs including care plans medication charts and all other relevant documents.
> High level of courteous communication and consultation with all stakeholders.
> Successfully lead staff in a multi-disciplinary environment promoting excellence in the standard of care provided for residents.
> Comply with reporting requirements for Quality Indicators Accreditation Internal Audits and any other reports as directed.
> Document and follow up all incidents and concerns.
> Implement emergency infection control procedures when required.
> Maintain a commitment to continuous improvement
> Comply with the Aged Care Service Policies Procedures Aims and Objectives adopted by Council and act in accordance with the code of professional conduct and Councils Code of Conduct.
> Promote a professional image for all facilities operated under the auspice of Gilgandra Shire Council.

QUALITY CARE
> Lead and actively participate in the delivery of a high standard of physical and emotional support and care to each resident and their families/friends.
> Assist residents to make informed decisions and achieve meaning and purpose in their lives.
> Responsible for Quality management data collection and evaluation including mandatory Quality Indicators data.

CLINICAL LEADERSHIP
> Provide a high standard of ongoing clinical management
> Monitor team clinical performance
> Lead the Team Leaders and EENs and mentor them in leading their teams.
> Plan and allocate work for the team
> Oversee clinical education e.g. wound/medication management competency assessments and monthly training programs.
> Prepare for medical officer visits and document resident outcomes

INFORMATION TECHNOLOGY
> Implement new systems for improvement adopted by management
> Update according to incoming publications/information
> Circulate publications
> Identify information source(s) inside and outside the organisation

COMMUNICATION
> Organise meetings eg Team Leaders Care Staff Residents and/or families
> Obtain relevant data from external sources
> Produce reports
> Establish and maintain networks
> Identify document requirements

ORGANISATIONAL DEVELOPMENT
> Assist with the development of future strategies
> Assist with planning to match future requirements with resources

> Display professional ethical positive and consistent behaviour within the work environment at all times.
> Participate in quality improvement programs to improve the lives of residents and staff
> Promote and develop a culture in the team which contributes to a harmonious and supportive work environment where people feel valued

SELECTION CRITERIA
ESSENTIAL CRITERIA
> Current registration with AHPRA
> Demonstrated ability to manage and supervise staff
> Demonstrated ability to work as part of a team.
> Proven record of quality care delivery
> Commitment to WHS & EEO principles.
> Excellent communication skills both written and verbal.
> Proficiency in use of Microsoft suite of applications
> Ability to provide on-call assistance after-hours
> Current Class C Drivers Licence

DESIRABLE CRITERIA
> Knowledge of ACFI
> Understanding of Aged Care legislation and regulations
> Understanding of Aged Care Accreditation standards
> Recognised qualifications in Management or equivalent experience
> Certificate in Aged Care or equivalent
> Ability to complete documentation requirements for an aged care environment
> Experience working with and empathy towards the elderly.
> Proven care plan experience
> Current First Aid Certificate

WORK HEALTH AND SAFETY

Expected Salary: $95000-$100000 AUD per year
