As a Complex Needs Specialist Practitioner you will provide holistic case management to families of children with complex disability needs supporting safety wellbeing and stability. You will undertake therapeutic assessments develop individualised Family Action Plans and assist families to navigate the NDIS while advocating for appropriate supports.
Working collaboratively with multidisciplinary professionals you will implement behaviour and parenting strategies coordinate care teams and broker services aligned to family needs. You will also promote the voice of children and young people maintain accurate records participate in supervision and ensure high-quality compliant service delivery through strong stakeholder engagement and adherence to organisational standards.

You will meet the person specification in the position description including having a degree in Disability Social Work Psychology or equivalent.
You will also have:
- Current drivers licence valid in Victoria
- Demonstrated ability as a practitioner and case manager in the disability child youth and family welfare field.
- Demonstrated experience engaging with families and successfully providing outreach services sometimes in a difficult or stressful context.
- Demonstrated experience supporting children and young people with complex needs related to their disability implementing positive behaviour support
