Your opportunityAs a Care Partner youll be a trusted advisor and advocate supporting clients to navigate services and funding so they can maintain their independence and quality of life in their own make an impact byConducting holistic assessments and developing personalised care plans aligned to each clients goals Building strong meaningful relationships with clients families and stakeholders Coordinating services and responding proactively to changing needs Supporting clients to understand and effectively utilise their budgets and funding Managing competing priorities in a fast-paced environment while maintaining high-quality outcomesThis role will suit someone who is resilient adaptable and confident navigating challenges with a genuine passion for helping youll bringTo succeed in this role youll need the following qualifications:Tertiary qualifications in the Human or Behavioural Sciences Allied Health Aged Care or Community Services FieldsExperience in client case managementNational Police checkDrivers licenceWe would also love you to bring the following skills:A natural ability to connect and build relationships with diverse individuals Genuine empathy and respect for different backgrounds and life experiences Strong communication skills with clients families and colleagues Organisation and the ability to manage competing priorities Resilience and adaptability in a fast-paced evolving environment Confidence (or willingness to learn) in supporting client budgets and funding A proactive solutions-focused mindset A collaborative approach and strong team focusWhy join ECHWe want you to be MORE with us.
